Uterus-Preserving Fibroid Treatment: Questions Before Pregnancy

Dr Harshita Kakarla MBBS, MS, FMAS
    Uterus-Preserving Fibroid Treatment

Uterus-preserving fibroid treatment may be considered when a woman wants to manage fibroids while keeping the uterus for future pregnancy. The right option depends on the fibroid’s size, number, location, symptoms, effect on the uterine cavity, age, fertility plans and overall health. Myomectomy is one uterus-preserving option. Fibroid location and whether the uterine cavity is distorted can matter more than size alone when fertility is being considered. Uterine artery embolisation and other non-surgical options require careful fertility counselling because their effects on future pregnancy can be uncertain. Myomectomy removes fibroids while leaving the uterus in place and may be considered for selected women planning pregnancy. Uterus-preserving fibroid treatment focuses on treating the problem while retaining the uterus whenever medically appropriate. This guide explains the questions to discuss with your gynaecologist before choosing treatment.

    Planning Pregnancy?
Call Talk to us. Talk to us.

    Can Fibroids Affect Future Pregnancy?

Yes, some fibroids can affect fertility or pregnancy, but many women with fibroids can become pregnant. The effect depends on factors such as the fibroid’s location, size, number and whether it changes the shape of the uterine cavity. Fibroids are non-cancerous growths that develop in or around the uterus. Some cause heavy menstrual bleeding, pelvic pressure or pain, while others produce no symptoms.

When fertility is the priority, doctors usually pay particular attention to whether a fibroid:

    • Protrudes into the uterine cavity
    • Changes the shape of the cavity
    • Is located within the uterine muscle
    • Is large or numerous
    • Causes significant symptoms
    • Is associated with fertility problems or pregnancy complications

Not every fibroid needs to be removed before pregnancy.

What Should You Ask Before Choosing Uterus-Preserving Fibroid Treatment?

First, ask whether the fibroid actually needs treatment before pregnancy. Your treatment decision should consider both your reproductive goals and the fibroid’s characteristics.

    1. Does My Fibroid Affect the Uterine Cavity?

A fibroid that distorts the uterine cavity may matter more for fertility than one that does not. The uterine cavity is the space where an embryo implants and develops. Submucosal fibroids and some intramural fibroids that change the cavity contour may therefore receive particular attention during fertility planning. However, the evidence is not strong enough to recommend removing every fibroid in every woman.

    2. Where Is the Fibroid Located?

Fibroid location can influence both treatment selection and fertility discussions.

    Doctors commonly describe fibroids as:

    • Submucosal: close to or projecting into the uterine cavity
    • Intramural: within the muscular wall of the uterus
    • Subserosal: projecting toward the outer surface of the uterus

The same-sized fibroid can have different clinical significance depending on where it is located. This is why you shouldn’t interpret a scan report based only on the fibroid’s size.

    3. How Many Fibroids Do I Have?

The number of fibroids can affect treatment complexity and surgical planning. A woman with one small fibroid may have very different treatment options from someone with multiple large fibroids. Your gynaecologist may review previous ultrasound reports and recommend further imaging if the exact number, location or relationship to the uterine cavity is unclear.

    Is Myomectomy a Uterus-Preserving Treatment?

Yes, myomectomy removes fibroids while leaving the uterus in place. It is therefore an important option to discuss with women who wish to retain their uterus and may want to become pregnant. At Lux Hospitals, myomectomy options include laparoscopic, hysteroscopic and abdominal approaches, depending on the size, number and location of the fibroids.

    Laparoscopic Myomectomy

Laparoscopic myomectomy uses small abdominal incisions and a camera-guided surgical approach. It may be considered for selected fibroids that can be safely removed through minimally invasive surgery.

    Hysteroscopic Myomectomy

Hysteroscopic myomectomy is performed through the vagina and cervix without external abdominal incisions. It is generally used for selected fibroids that project into the uterine cavity. ACOG identifies hysteroscopy as an approach for fibroids that protrude into the uterine cavity.

    Abdominal Myomectomy

Abdominal or open myomectomy may be considered when fibroids are very large, numerous or difficult to remove safely using minimally invasive techniques. The aim remains to remove the fibroids while preserving the uterus when appropriate.

Is Uterus-Preserving Fibroid Treatment Always the Same as Fertility-Preserving Treatment?

No. Preserving the uterus and preserving fertility are related but not identical goals. A treatment may leave the uterus intact but still have uncertain effects on future fertility or pregnancy. For example, ACOG states that the effect of uterine artery embolisation on future pregnancy is not clear, while more research is needed regarding radiofrequency ablation and future pregnancies. Therefore, women who actively want to conceive should tell their gynaecologist about their pregnancy plans before selecting a procedure.

    What About Uterine Artery Embolisation?

Uterine artery embolisation can shrink fibroids while preserving the uterus, but its effect on future pregnancy remains uncertain. Therefore, discuss it carefully when future fertility is a major priority. During UFE, tiny particles are delivered through the uterine arteries to reduce blood flow to the fibroid. The reduced blood supply causes the fibroid to shrink. It can be useful for selected women with symptomatic fibroids, but fertility goals should be discussed before treatment. NICE guidance specifically recommends counselling women considering pregnancy that the effects of UAE on fertility and pregnancy are uncertain.

Can Medication Treat Fibroids Before Pregnancy?

Yes, medicines can help control some fibroid symptoms, but they do not necessarily remove the fibroid. Depending on the situation, medication may help manage heavy menstrual bleeding or pain. Some hormonal treatments are not suitable when a woman is actively trying to conceive.

Therefore, tell your doctor whether you are:

    • Trying to conceive now
    • Planning pregnancy within the next year
    • Considering fertility treatment
    • Delaying pregnancy for several years
    • Unsure about your pregnancy timeline

Your pregnancy plans can change which treatments are appropriate.

What Tests Do You Need Before Choosing Treatment?

Ultrasound is commonly used to assess fibroids, while MRI may be considered when more information about their location, size or number is needed.

    Your evaluation may include:

    1. Medical history — symptoms, menstrual pattern, fertility history and previous treatments.
    2. Pelvic examination — when clinically appropriate.
    3. Ultrasound — to assess the uterus and fibroids.
    4. MRI — when additional anatomical detail is needed.
    5. Fertility assessment — when there are concerns about conception or other fertility factors.

What Important Questions Should I Ask About Myomectomy?

Before choosing myomectomy, ask your surgeon:

Will removing my fibroid improve my chances of pregnancy?

Not necessarily in every case. The potential fertility benefit is better established for selected fibroids that distort the uterine cavity, while evidence is less clear for fibroids that do not.

Which surgical approach is suitable for me?

The approach depends mainly on fibroid location, size, number and surgical complexity. Options may include hysteroscopic, laparoscopic or abdominal myomectomy.

    Could surgery affect my fertility?

Myomectomy preserves the uterus but, like any uterine surgery, can have risks such as bleeding, infection and scar tissue formation. Discuss your individual risks before surgery.

When can I start trying for pregnancy?

No single waiting period applies to every woman. The recommended interval depends on the type of surgery, depth of uterine repair and individual healing, so your surgeon should provide a personalised timeline.

Could I need a caesarean birth after myomectomy?

Possibly. The need for caesarean delivery depends on factors including how deeply the uterus was incised and repaired during surgery. ACOG notes that some women who become pregnant after myomectomy may need caesarean birth.

What Are the Risks of Uterus-Preserving Fibroid Treatment?

Every treatment has potential benefits and risks, so fertility goals should be part of the decision.

    For myomectomy, possible risks include:

    • Bleeding
    • Infection
    • Adhesion or scar formation
    • Injury to nearby organs
    • Anaesthesia-related complications
    • Fibroid recurrence
    • Possible effects on future pregnancy depending on the uterine repair

Myomectomy removes existing fibroids, but new fibroids can develop later.

A good consultation should cover both your short-term treatment goals and your longer-term reproductive plans.

How Long Does Recovery Take Before Planning Pregnancy?

Recovery varies according to the type and extent of treatment. Hysteroscopic and laparoscopic procedures may have shorter recovery periods than open abdominal surgery, but the time before attempting pregnancy is a separate clinical decision. Lux Hospitals describes different myomectomy approaches based on the fibroid’s size, number and location. Do not use the date you feel physically better as the date to start trying for pregnancy. Your uterus may need additional healing time even after you return to normal daily activities.

How Much Does Uterus-Preserving Fibroid Treatment Cost in Hyderabad?

The cost depends on the treatment approach, fibroid characteristics, hospital stay and postoperative care. A current Lux Hospitals treatment page lists prices, while the final estimate depends on the surgical approach and individual case.

    Cost factorWhy it matters
Fibroid size and numberMore complex cases may require more extensive surgery
Fibroid locationLocation influences the surgical approach
Laparoscopic vs open surgeryDifferent equipment and hospital requirements apply
    AnaesthesiaDepends on the procedure
Hospital stayMay vary according to the surgery and recovery
InvestigationsUltrasound, MRI and blood tests may add to the overall cost

Ask for an individual estimate after your scan and consultation rather than relying on an online price alone.

    Frequently Asked Questions

Yes, Many fibroid-affected women can conceive. However, their location and impact on the uterus should be evaluated since some fibroids, especially those that deform the uterine cavity, may interfere with conception or pregnancy.

It can be appropriate when preserving the uterus is an important goal, but there is no single best treatment for everyone. Myomectomy is a uterus-preserving option, while the suitability of other procedures should be discussed carefully when future pregnancy is planned.

No. Treatment is not automatically required simply because a fibroid exists; symptoms, size, location, cavity distortion and fertility history all influence the decision.

Fibroids that distort the uterine cavity are the most important to evaluate when fertility is a concern. ASRM reports evidence that removing cavity-distorting fibroids can improve pregnancy rates in selected women.

Its effect on future fertility and pregnancy remains uncertain. Women planning pregnancy should discuss this specifically with their gynaecologist before choosing UAE.

Yes. Lux Hospitals has gynaecologists and advanced laparoscopic surgeons who manage uterine fibroids and uterus-preserving procedures. Dr. Harshitha Kakarla's profile lists advanced laparoscopy and operative hysteroscopy among her areas of expertise.
